Coming home to Hamilton

Whether your family member is coming home from Hamilton General, Juravinski Hospital, St. Joseph's Healthcare or McMaster University Medical Centre, call us as soon as a discharge date is likely. Care can then be planned before they leave, not after.

If a hospital care coordinator arranges publicly funded home care through Ontario Health atHome, our private visits can be scheduled around those visits to cover the hours in between.

Common Questions

Hospital-to-Home Care: Common Questions

When should I call about hospital discharge?

As soon as a discharge date is being talked about. Even a day or two gives us time to plan the first visits and have a caregiver there when your family member gets home.

Do you work with Ontario Health atHome?

We're a private home care provider. If your family member has been approved for publicly funded care, we can add private hours around those visits so there are no gaps.

How quickly can care start?

Often within a few days of your consultation, and sometimes sooner. If you need help urgently, for example for a hospital discharge, call us at (226) 506-7013 and we'll tell you straight away what we can arrange.

How much does home care cost?

It depends on the type of care and how many hours you need. At your free consultation we'll recommend a care plan and tell you exactly what it will cost before anything starts, with no obligation. Some private insurance plans and Veterans Affairs Canada programs help with the cost of home care, so it's worth checking your coverage.

Will we have the same caregiver?

We aim to keep the same caregiver, or a small consistent team, for every client. If a change is needed, we tell you ahead of time and introduce the new caregiver.
